The "Cardiac/Diabetes TRANS-CARE program", which consists of two face-to-face sessions, a home visit and telephone follow-up. Purpose of the intervention: to increase self-efficacy to enhance self-management behaviours and thus improve health outcomes and reduce hospital readmissions.

The purpose of this study is to develop a new self-management program based on a self-efficacy model and to test its effectiveness for adolescents with asthma in Taiwan. .
The results of this study will answer the following questions.
What is the effectiveness of an asthma self-management program in comparison to usual care on: self-efficacy, asthma self-management behaviors, and symptoms of asthma?
What is the relationship between self-efficacy and asthma self-management activities?
What is the relationship between asthma self-management behaviors and outcome-expectation?

The purpose of this study was to 1) To revise and translate an integrated Cardiac-Diabetes Self-Management Program (CDSMP) incorporating telephone and text-messaging into Chinese. 2) To evaluate the effectiveness of CDSMP for patients with type 2 diabetes who have suffered a critical cardiac event among Australian and Taiwanese populations.

People with cardiac disease who have type 2 diabetes have higher readmission rates (22%) than those without diabetes (6%) (Wu & Chang, 2008). Although self-management is an effective approach to achieve better health outcomes, there are no such programs specifically designed for patients with both conditions. Such programs need to be enhanced by user-friendly technology and supported by patient peers. This project aims to extend the development of a peer support based Cardiac-Diabetes Self-Management Program (CDSMP) by preparing a DVD to support persons' engagement in follow-up study.
