Abstract

This toolkit includes pre-implementation establishment information for creating a role for an onsite Nurse Practitioner in an aged care facility or care home and the Geriatric Emergency Department Intervention (GEDI) model of care in the Emergency Department of a hospital. The results of the evaluation of this model are included showing how these interventions coordinated care for older adults in aged care facilities.
