Treatment approaches to Mycobacterium abscessus pulmonary disease

Abstract
Mycobacterium abscessus complex comprises three subspecies. Subspecies abscessus and bolletii usually exhibit inducible macrolide resistance, whereas subspecies massiliense usually does not. Macrolide susceptibility is determined by phenotypic testing after 14 days of incubation. Macrolide susceptibility is the key determinant of treatment outcome for M abscessus pulmonary disease (MAB-PD) and must be protected by adequate companion drugs in treatment regimens. The optimal approach to treatment of MAB-PD is undefined. Current treatment strategies result in poor outcomes, especially for macrolide resistant isolates, and considerable drug toxicity. Multiple novel or repurposed therapeutic candidates are emerging.
