International and cross-agency collaboration to improve child death review: making every child count

Abstract
The detailed investigation of SUDI cases has shown how much can be learned from infant deaths, and this learning used to prevent future deaths. As a result of the success of SUDI investigations, many countries have widened their scope of investigation to include child deaths from other causes, this detailed scrutiny is called Child Death Review (CDR). In some countries, (eg England), all child deaths are reviewed, others (eg USA) reviews focus on unexpected or unnatural child deaths. Several countries now have registries or databases for recording causes, risk factors, and learning points from child deaths. Many countries also have review systems for deaths relating to child abuse or neglect.
Although the structure of CDR is different between countries a common aim is to learn from child death and reduce future mortality. CDR may identify factors in individual children, society or health care services contributing to deaths, and learning from deaths should not be limited by international borders. Understanding how different CDR systems work, their challenges and success stories will promote effective CDR practice locally and enable consideration of joint international CDR projects.
We will present 4 short abstracts on CDR in England, USA, Australia and France; followed by a panel discussion on exemplars of good practice, implementation strategies and future collaborations for best practice.
