Inclined Infant Sleep Environments: A Balancing Act in Driving Behaviour Change Across the Sector

Abstract
Sudden Unexpected Deaths in Infancy (SUDI) remains the largest category of post-neonatal deaths in Australia. ‘Sleep baby on the back from birth, not on the tummy or side’ remains a key recommendation underpinning the national campaign, while a firm, flat mattress is advised under sleep environment messages. Globally, inclined sleep surfaces have been associated with a considerable proportion of SUDI deaths often involving nursery products or makeshift bedding arrangements. Inclined sleep increases risk of hazardous chin-to-chest positioning and infant airway compromise, and propensity of the infant to roll into non-supine positions or hazardous surfaces. Evidence suggests that parents continue to experience challenges in implementing supine sleep on a firm, flat surface while misinformation about inclined sleep positions, particularly for premature and unwell infants, continues to permeate.
Objectives: This symposium will present a systems approach in which learnings from child death review, and caregiver reports of the utility of current safe sleep messaging approaches, have informed contemporary safe sleep guidelines which directly address inclined sleep practices in a variety of clinical and community settings. Provisions for infants with special circumstances are made. Recommendations relevant to nursery product regulators and suppliers will be discussed, with emphasis placed on the potential of cross-sectoral collaborations to reduce infant deaths. Clarity in revisions of sleep position messaging to specifically address inclined sleep positioning advising Sleep baby on the back, on a firm, flat, level surface’ will be highlighted.
