Background: NSF National Stroke Guidelines aims for both the stroke survivor and family/carer to be consulted throughout the acute, rehabilitation and discharge planning phase. Aim: This consumer presentation has two aims: • The aim of this presentation is to provide insight from the stoke survivor and carer perspectives, on care integration. • To stimulate thinking for future directions of planning care. It is not the aim of this presentation to plug up the holes but to stimulate thinking. Method: Consumers stories are, are used to explore the following questions: • What have been the positive memories of the planning of your care throughout your stroke journey? • Have you had any negative experiences? If yes, could you discuss these experiences? The vod cast narratives provide a perspective to understand positive and negative lived experiences of care planning in the stroke journey. Results: Participants felt disempowerment in the planning and delivery of care throughout the stroke journey. They highlighted a lack of integration of care as a key barrier to receiving long term quality care. Conclusion: Individual health professional within the health system strive to provide a "person centred" approach but stroke care is largely driven by cost, resources and the health system.
